Tri-County Project Care Update

Since the generous funding and efforts of our supporters to serve the growing uninsured in our area and the subsequent January opening of Tri-County Project Care, the program has exceeded the original expectations.
Currently, 270 patients are enrolled with an additional 25 enrollees scheduled for registration.
Together the Charleston County Medical Society, the Medical Society of South Carolina, and the federal
health centers have enabled us to find a medical home for each enrollee. As a result, TCPC is meeting
the challenge of decreasing the burden of care for our area emergency rooms and promoting
wellness through affordable access to quality care.
Meeting the Demand
In order to meet the growing demand for care, Elizabeth McCaleb, Program Coordinator has recruited
three new case managers. We welcomed Rose Goodwater, BSW in February, and Lois Montague and Sherry
Power in May of this year. Evelyn Thomas, Administrative Assistant, is the central reception for
all enrollment calls at 843-853-3131. Our rapidly growing team is enrolling uninsured, employed
residents as quickly as available funding will permit.
New Location
We have moved our office to more centrally serve the three counties to the United Way building on
Rivers Avenue. Our new address is 6296 Rivers Ave, Suite 309, North Charleston, SC, 29406.
Phone numbers remain unchanged.
Centralized Link
Our social work staff commits significant time and energy guiding and referring callers within
the community. TCPC has linked over 900 callers to resources ranging from federal programs and
employment services to neighborhood outreach initiatives and support groups. Together with our
community partners, TCPC is bridging the underserved and uninsured gap in our community by providing
a centralized link to all area health related services.
Outreach
In an effort to reach out to our patient base, the TCPC case managers will be seeing patients
for outreach locations including DHEC Moncks Corner, DHEC Goose Creek, Franklin C. Fetter downtown,
Northwoods Mall, and Sea Island Comprehensive Heathcare.
Duke Endowment
We are pleased that the Duke Endowment has recognized the TCPC program as a worthy cause by
granting 750,000 dollars toward raising awareness, outreach and administrative expenses.
The Media
The media has taken a special interest in our program. Over the last several months we have
enjoyed significant coverage in the Post and Courier, WCIV Channel 4, WCSC Channel 5, WCBD Channel 2
News, WCBD Channel 2 "To Your Health" live feature, all the weekly papers, and the
Citadel radio stations. On each occasion we expressed our gratitude to the TCPC sponsors that
made all the good we are doing each day possible.
Next Steps:
- Focusing the efforts of our board and advisory committee
- Raising awareness regarding key health issues among our constituency
- Increasing the enrollment of new patients and forging new beneficial partnerships
- Continued involvement in related legislation
- Enhancing the prescription and psychiatric programs
- Developing an outcomes tool to measure TCPC impact
- Pursuing federal and corporate funds to cultivate our program to its full potential
